• Completed application, including signature and date
• Copy of your current teaching certificate
• Official transcript(s) with grades posted for ALL undergraduate/graduate courses received directly from the college/university attended. (You may include photocopies of transcripts with your application while official transcripts are being ordered.)

Note: If your cumulative GPA is less than 3.0, you must include two (2) letters of recommendation.

Note: If you are not certified, but teach in a private/Catholic school, you may supply one of the following documents instead of a teaching certificate:

• Letter of recommendation from your principal
• Copy of your contract
• Copy of your pay stub
